Dept. of Elder Affairs Seeking Home Health Provider Input on COVID-19 Florida Safe Survey

HCAF, in partnership with the Florida Department of Elder Affairs (DOEA), would like to encourage home health providers to take a moment to take the anonymous online self-assessment at FloridaSafeSurvey.com and receive your Safe Behavior Report.
DOEA recently launched the Florida Safe Survey as a helpful self-assessment tool allowing older adults to better assess their behaviors during COVID-19. This tool will further their efforts to slow the spread of COVID-19 and be a resource to Florida's older adult communities.
Upon completing the survey, each respondent will receive a Safe Behavior Report, which offers customized recommendations and provides you with links to useful, curated resources to address self-identified needs such as transportation, medication, food, social services, health care, finances, and other issues. In addition, the survey may be taken as many times as you wish to see how any changed behavior may affect your Save Behavior Score.
As COVID-19 precautions continue to evolve throughout the state, DOEA wants all Floridians, especially high-risk individuals, to have a tool to make the most informed personal decisions regarding safety. This survey is intended to do just that — to help protect older adults, their caregivers, and families as COVID-19 continues to unfold.
The survey is available 24/7 on computers, tablets, and mobile devices.
